Despite bundled payments initiatives and ACO approaches, health care operates in a silo. That is no more apparent and important than in the care of our elders. What happens during transitions of care and in the eighty percent of care that is impacted by social determinants of health is more important than the twenty percent of care in hospital and practice settings. Agiing expert and ACHE Fellow Anthony Cirillo will connect the dots in aging care.
